Instructions
CSLA has contracted with the DoubleTree Hotel, the Ayres Suites, and the Country Side Suites. The hotels are adjacent to the convention center. The Doubletree has been designated as our conference headquarters and has just finished extensive remodeling.

Hotel reservations are being handled by The Ontario Housing Bureau. Reservations are not limited to conference dates. Reservations will be accepted two days prior and two days after the conference if needed. There are two ways to register, either online or by printing a blank reservation form and mailing or faxing the completed form. Online and faxed forms must be accompanied by a credit card number. Mailed forms may include a check. Sorry, no telephone reservations or purchase orders are accepted. Complete the entire form to ensure speedy processing. If sharing a room, designate one person to send this request. One form per room, please. Reservations are accepted on a first come, first served basis. Prior to processing your housing request, all required information must be completed correctly. All deposits, either check or credit card, must be accompanied by the housing reservation form.

Deadline
All reservations must be made and received by the Housing Bureau by Oct. 25, 2005. After that date, based on hotel availability, the Housing Bureau will continue to process housing reservations until 4:30pm (PST), Oct. 31, 2005. Rates may vary from the special CSLA rates in effect prior to the Oct. 25, 2005 deadline. After Oct. 31, 2005, please contact hotels directly for new room reservations, changes, or cancellations.

Deposit
All reservations must be guaranteed by providing a major credit card or a deposit of $200 made payable to OCVB. A credit card is the preferred method to guarantee your reservation. Your credit card is only a form of guarantee and will not be charged unless you fail to advise us of your cancellation. If you do not arrive or cancel within 24 hours of your arrival date, you will be liable for the first night's deposit.

Modifications / Cancellations
All reservation changes or cancellations must be made in writing or emailed to the Housing Bureau prior to Oct. 31, 2005. The assigned hotel must receive room cancellations at least 24 hours prior to arrival for refund.
