Instructions
CSLA has contracted with the DoubleTree Hotel, the Ayres Suites, and the Country Side Suites. The hotels are adjacent to the convention center. The Doubletree has been designated as our conference headquarters and has just finished extensive remodeling.

Hotel reservations are being handled by The Ontario Housing Bureau. Reservations are not limited to conference dates. Reservations will be accepted two days prior and two days after the conference if needed. There are two ways to register, either online or by printing a blank reservation form and mailing or faxing the completed form. Online and faxed forms must be accompanied by a credit card number. Mailed forms may include a check. Sorry, no telephone reservations or purchase orders are accepted. Complete the entire form to ensure speedy processing. If sharing a room, designate one person to send this request. One form per room, please. Reservations are accepted on a first come, first served basis. Prior to processing your housing request, all required information must be completed correctly. All deposits, either check or credit card, must be accompanied by the housing reservation form.

Deadline & Cancellations
All reservations must be made and received by the Housing Bureau by October 29, 2007. All changes or cancellations must be in writing or e-mailed to the Housing Bureau by 4:30 pm October 29, 2007. After October 29, for new room reservations, cancellations or changes, call the DoubleTree Hotel (800) 222-TREE or the Ayres Suites/Country Side Suites (800) 248-4661 directly. Rates may vary from special CSLA rates in effect prior to the October 29, 2007 deadline.

Deposit
All reservations must be guaranteed by providing a major credit card or a deposit of $200 per room reservation by check, made payable to Ontario Housing Bureau. Reservations not guaranteed will not be processed. If you do not arrive or cancel within 24 hours of arrival date, you will be liable for the first night’s deposit. Reservations are processed on a first-come, first-served basis. Purchase orders are not accepted.
